Preparing for your first visit

Before your first visit, you will be asked to fill out a Confidential Health Questionnaire, Assumption of Risk and Release of Liability Waiver, and a HIPAA form.

Expect to spend about an hour to an hour and a half with the counselor reviewing your health history, current health struggles, recent labs, current diet, and more. This is a chance for you to share your story and where you want to go on your health journey.

Together, the client and counselor will begin to investigate the root cause of symptoms and brainstorm goals that will fit into the wellness plan. The client should not expect any formal recommendations during the first visit.

We will discuss these in the 20-minute follow up (included in the initial consultation) or the next 45-minute follow up (as part of the 3- or 6-month package).

Schedule a FREE 15-minute discovery call. During this call we will discuss your concerns and determine in what capacity we might work together. Next you will schedule your initial consultation.

Intake forms

Telehealth meetings

Initial and follow-up consultations are typically done over video conferencing for a comfortable and authentic working relationships. Phone call meeting are also an option if that is more convenient.

Goal setting

Working toward long-term and short-term goals is an essential key to success. Regular goal setting helps you focus your efforts on what is important to you and what you want to change.

Follow up support

Through email, text, and/or follow up video calls, we provide additional encouragement, accountability, and nutrition education to help you overcome roadblocks in order to meet your goals.

Lab testing

It is helpful for the client to provide basic blood work in order to assess nutritional needs. Additional functional lab testing might be recommended in certain instances in order to provide more specific information about a condition or need.
